Incoming Student Forms

All incoming full-time undergraduate and graduate students under 30 years of age and ALL full-time and part-time health science and International student must complete the following forms:

Physical Examinations

All new students must have had a physical examination on or after September 1, 2022 (or two years prior to the beginning of the semester). If you are not yet due for your insurance-covered annual physical, you're welcomed to schedule a physical exam appointment free of charge at the Health Center.

Student-Athlete Forms

Any new NCAA student-athlete must have a physical exam done within six months before participation in any practice, competition, or out-of-season conditioning activity.

Every incoming and returning student-athlete must also complete additional forms required by athletic training. Your head coach or head athletic trainer will have the password for each team. You have to be on a roster first to access the electronic medical record system. To get on a roster, contact the head coach.

Please note that these requirements are separate from the incoming student Health Center requirements, which must be on file with the Health Center.

Accessibility and Accommodations

The Academic Success Center’s Disability & Accessibility Services (DAS) is the College’s designated office for determining eligibility and facilitating student access in academic settings and non-academic (i.e. housing, dining, transportation, etc.) accommodation requests.

Registration holds have been placed on students who are non compliant with Springfield College health requirements
